People install two of it :-)
Either they already have layer 3/4 load-balancers, and they put two haproxy boxes behind them, or they install an IP take-over daemon such as keepalived on linux or CARP on BSD so that at any moment, the service address keeps being served.
Take a look at the architecture manual, there are examples of such setups.
